Outpatient Rehabilitation

Outpatient services are a part of Euclid Hospital's seamless continuum of rehabilitation care. This provides the option of acute inpatient care, subacute care, comprehensive inpatient rehabilitation, comprehensive outpatient rehabilitation, individual rehabilitation therapies, behavioral health services, home health and follow-up care.

Outpatient Rehabilitation Facility
Euclid Hospital houses an exercise therapy pool, work simulation devices, the latest exercise equipment organized in special orthopedic, neurologic and aqua therapy treatment areas. Programs include: stroke rehabilitation, outpatient physical, occupational, speech therapy and audiology services, neuro day hospital program, wound clinic, lymphedema program, pediatric therapies, audiology services, drivers' simulation and on-the-road testing, balance program, and hand clinic.

 

 

 

Neuro Day
The Neuro Day Hospital Program at Euclid Hospital's Health Center provides a comprehensive, coordinated continuum of care. This skilled care program is designed but not limited to individuals with neurological impairments. The patient no longer requires 24 hour inpatient medical care but still requires intensive rehabilitative therapy and occasional nursing intervention. Other diagnoses are accepted on a case-to-case basis. The Neuro Day Hospital Outpatient Program averages five weeks and provides 3 full days per week of skilled intervention.

Referrals to other appropriate agencies or professionals may be made as necessary.

 

Treatment Program
An individualized treatment program is established for each patient utilizing a combination of individual treatment sessions and group therapy. Specifically designed treatment groups are provided by each discipline. The Neuro Day Program specializes in strengthening the individual patient from a social and psychological perspective. Each patient is regularly reviewed by the multi-disciplinary team including the patient's physiatrist.

 

Admission Criteria and Referral Process
The Neuro Day Hospital Program accepts patients with a neurological diagnosis such as stroke. The referred should be medically stable and have a reasonable expectation for further improvement. Other diagnoses (e.g. neuromuscular disorders, arthritis and degenerative disease) are accepted on a case-by-case basis. Patients must require a minimum of two therapy disciplines and benefit from a therapeutic group environment.

 

A physician's prescription is required for services. Referrals can be made directly by case managers, social workers, physicians, therapists or family members. Please call (216) 692-7476. Preliminary information will be obtained to determine patient eligibility.

 

In our effort to assist individuals and their families, the Neuro Day Hospital Program offers a monthly Stroke Education Support Group and a Caregiver Support Group. Please contact (216) 692-7476 for more information.

 

Back & Neck Care Center
Most adults will experience at least one episode of disabling back or neck pain in their lifetime.

The Back & Neck Care Center, located on the Euclid Hospital campus, can help alleviate your pain and discomfort. Our mission is to provide an appropriate, comprehensive and cost effective approach to caring for your back and neck.

Insurance
Back & Neck care services are usually covered by major insurance plans, as well as worker's compensation, Medicaid and Medicare. Some insurance carriers require a referral from your primary care physician. We will assist you with your insurance options.
