Congestive Heart Failure

The CHF and CKD Center at Euclid Hospital is committed to improving the quality of life for people who have Congestive Heart Failure and Chronic Kidney Disease.  By encouraging patients to actively participate in their own medical management of their disease, the professional staff helps individuals control their disease, improve their quality of life and avoid hospitalizations. 

The education and treatment program includes: 

  • Close monitoring of cardiac status
  • Education in disease, diet, exercise, medication, signs and symptoms
  • Fluid and electrolyte management
  • Support for patients and their families
  • IV medication
  • Lab assessment
  • Cardiac monitoring

Euclid Hospital has a Congestive Heart Failure and Chronic Kidney Disease Support Group for patients, as well as their families, caregivers and other loved ones. Click here for more information.
