One out of every nine Americans has this disease and another 20 million people are at risk.  Through early detection and treatment, medical professionals can often keep the disease from worsening. 

The CHF and CKD Center at Euclid Hospital is committed to improving the quality of life for people who have Congestive Heart Failure and Chronic Kidney Disease.  By encouraging patients to actively participate in their own medical management of their disease, the professional staff helps individuals control their disease, improve their quality of life and avoid hospitalizations. 

The Education and Treatment Program includes:

  • Close monitoring of renal status
  • Education in diet, exercise, medications, and signs and symptoms
  • Hypertension, fluid and electrolyte and bone disease management
  • Anemia treatment
  • Support for patients and their families
  • Cholesterol management
  • Kidney bone disease management
  • Continual follow-up
